PRINT THIS FORM MODEL RELEASE FORM In consideration of my/our Engagement as a model, I hereby grant Naughty Soap / MSP Media (Photographer) any and all rights and those they represent, authority and permission to use and publish photographic pictures, video tapes, and other media used to capture my likeness, or in which I may be included in whole or in part, in conjunction with my fictitious name, reproduction thereof in color or otherwise made any media at their studios or elsewhere, for art. I also consent to use of any printed matter in conjunction therewith. I consent to printed matter as stated true or fictitious. I hereby release, discharge and agree to save harmless, Photographer and their legal representatives or assigns, and all persons acting under their permission and authority or those for who they are acting, from any liability by any virtue or any blurring, distortion, ever. Also included are alterations, optical illusions, or use in composite form or otherwise, that may occur or be reproduced in the taking of said photographs and said video or in any subsequent processing thereof, as well as any publication thereof at all. During the photography session I did realize I was being photographed or videotaped, and I affirm that Photographer did this freely and in good will. I do understand I will be paid a fair rate in amateur fees by Photographer for appearing in said video or photographs. I hereby give Photographer the representatives and assigns, those for whom Photographer is acting and those acting with Photographers' permission employees, the right and permission to copyright and / or use, reuse and / or publish, and republish video images or photographic pictures or portraits of me, or which I may be distorted in charter, or form, in conjunction with my fictitious name, on reproduction thereof in color or black and white made through any media by Photographer or elsewhere, including in published material, books, electronic media, Web sites, the Internet as a whole, or any other media, for any purpose whatsoever, including the use of any printed matter in conjunction therewith. I also understand and agree that my images may or may not be used in any matter at all, at the sole discretion of Photographer. I further release Naughty Soap / MSP Media, or others acting on its behalf, from any claims for remuneration associated with any damage, foreseen or unforeseen, associated with the proper commercial or artistic use of these images unless it can be shown that said reproduction was maliciously caused, produced and published for the sole purpose of subjecting me to conspicuous ridicule, scandal, reproach, scorn and indignity. I acknowledge that the photography session was conducted in a completely proper and professional manner, and this release was willingly signed at its termination.
